蒐集個人資料告知事項暨個人資料提供同意書蒐集個人資料告知事項

臺北市產業獎勵專案辦公室為遵守個人資料保護法規定，在您提供個人資料前，依法告知下列事項：

臺北市產業獎勵專案辦公室為提供辦理計畫說明會、研習課程、成果發表會、一對一指導快速送件服務、產業推廣輔導或政令宣導活動報名相關等等特定目的，並確保報名人員之利益，將遵循個人資料保護法及相關法令之規定，蒐集、處理及利用報名人員所提供之個人資料，包括服務公司商號、公司營登號碼、部門、職稱、姓名、電子郵件帳號、通訊住址、聯絡電話及手機等，於蒐集目的之存續期間合理利用您的個人資料。
您可依個人資料保護法第3條規定，就您的個人資料向本部行使之下列權利：
1. 查詢或請求閱覽。
2. 請求製給複製本。
3. 請求補充或更正。
4. 請求停止蒐集、處理及利用。
5. 請求刪除。
您因行使上述權利而導致對您的權益產生減損時，臺北市產業獎勵專案辦公室不負相關賠償責任。另依個人資料保護法第14 條規定，得酌收行政作業費用。若您未提供正確之個人資料，亦將無法為您提供特定目的之相關業務。
個人資料利用之期間、地區、對象及方式：
1. 蒐集方式：網路報名系統、本人紙本親寫，或以其他方式報名活動及課程者。
2. 保存及利用期間：您的個人資料將自報名日算起，保存3年，以做為本人、主辦機關、執行單位及臺北市產業獎勵專案辦公室查詢。
3. 個人資料使用之限制：本計畫依此聲明所蒐集之個人資料僅限計畫於蒐集目的及用途內使用，不會提供給計畫不相關之第三方單位使用。

提醒：
您得自由選擇是否提供個人資料或行使個人資料保護法第三條所定之權利，但報名提供資料不足或有其他冒用、盜用、不實之情形，將可能導致無法參加相關課程及活動，進而影響各項相關服務或權益。

Personal information collecting, processing and using statement

Personal Information Protection Act（hereinafter “this Law”）is enacted to govern the collection, processing and use of personal information so as to prevent harm on personality rights, and to facilitate the proper use of personal information. Project Office of Subsidies & Incentives for Taipei Industry will follow this Law when personal information is collected by conference, symposium...or any other means.
The personal information mentioned above refers to the name, occupation, company name, contact information, and other information which may be used to identify a natural person, both directly and indirectly.
All the personal information will not be collect, process or use beyond the original purpose.
Important note: You have the rights to choose whether to provide your personal information or not. However if the registration information is not complete or false, your rights to participate the activity or service may be effected.
